PRESCRIPTION REFILL
PLEASE READ THE
FOLLOWING
E-mail offers an easy and convenient way for
patient and doctor to communicate. In many circumstances it
has advantages over office visits or telephones. But
remember there are important differences. E-mail is not the
same as calling our office - there is no person at the other end
of the call - just a computer. You can't tell for
certain when your message will be read, or even if your doctor is
in the office or on vacation. Nonetheless, we believe the
ease of communication E-mail affords is a benefit to patient
care. Below are our rules for contacting us using
E-mail.
-- E-MAIL IS NEVER, EVER,
APPROPRIATE FOR URGENT OR EMERGENCY PROBLEMS!
-- E-MAIL is not
confidential. If you send a message from your place of
work, understand your employer has a legal right to read your
e-mail if they choose. System operators for most e-mail
systems have access to all of the e-mail that goes through that
machine.
-- We will NEVER disclose
your E-MAIL address to another organization without your
permission.
-- BE CERTAIN it is OK with
you that other friends/family having access to the e-mail address
you are using can read whatever response we feel is appropriate
to the message you send us.
-- The use of E-mail IS NOT
a substitute for a clinical examination. If you think you
might need to be seen by a physician or nurse practitioner - you
will - call and book an appointment!
-- If you would like to request a
current prescription refill from your physician simply fill out
the following form. We will confirm the details and phone
the refill into the specified pharmacy. Be sure to fill out
the request form completely.
